Membership Terms: Member services are valid for 61 days; from the sale date, services not redeemed in the allotted time will expire and be considered forfeited. We recommend scheduling appointments four weeks in advance for the best selection of time and service providers. Any member under the age of 18 must have a parent or legal guardian co-sign and be present for the initial visit. Rejuvalase Memberships may not be used in combination with any other promotional offers or packages, discount coupons, or gift certificates. No substitutions or refunds. Rejuvalase reserves the right to vary, add or eliminate any of the particular services provided from time to time. Members will receive the best value offered at the time the service is rendered.

Membership Benefits: Monthly members-only spa promotions, a discounted pricing additional purchase of menu priced products & menu priced services. Gift up to two of your yearly membership monthly experiences to a friend or family member for a transfer fee of only $10; *the same recipient may enjoy one transferred service every six months by the same Member. Bring a guest and share your member discount on their purchase of menu-priced services and products.

Payment Terms:  Membership must be purchased with a valid credit/debit card only. No other form of payment is accepted. All payments due will be collected by a valid debit or credit card, which the Member shall keep on file with the Rejuvalase. It is the responsibility of the Member to update/change all credit card information. Expiring credit cards, changing bank accounts, or lost or stolen cards must be updated before the following billing cycle.

Automatic Renewal Terms:  Memberships are auto-renewable and will automatically continue monthly until your Membership is canceled. Members who wish to cancel their Membership after the one-year initial membership period may do so by notifying Rejuvalase in writing (admin@rejuvalase.com) no less than thirty (30) days before the end of the then-current membership term. All Memberships renew at the sole discretion of Rejuvalase, which reserves the right to cancel this Membership Agreement at any time and to refuse to continue this Membership Agreement with Members.

Appointments & Late Policy: Our standard 24-hour cancellation policy applies to all members. If you miss, cancel, or change your appointment outside this designated time, you may be charged a cancellation fee for your scheduled services, or you may risk forfeiting your scheduled monthly benefits. Late arrivals may result in a shortened appointment or rescheduling (charges may apply). A no call, no show will result in a loss and forfeit of the scheduled monthly services.

Freeze Policy:  There may be circumstances when you would like to freeze your Membership due to extended illness, temporary relocation, or military leave. When a membership is frozen, a member may not use his/her membership benefits, or payments will resume that same day. The Membership can be frozen for up to 3 months in 12 months; a fee of up to $15 may be applied. Your membership expiration date will be extended by the amount of time that the Membership was frozen. Terms, conditions, and rules of the original membership agreement will continue to apply. A minimum of three (3) payments must be made before a membership is eligible to be frozen.

Fees:  If an automatic debit or credit card submitted by the Member is not honored or declined, then a fee of no more than $45 may be added to the account, in addition to any unpaid balance of this Membership Agreement. You may continue to redeem your membership privileges as long as your Membership is in effect and payments are current.

Cancellation:  There is no cancellation fee after twelve months. To cancel this Membership before the end of the contract, a $100 termination fee, plus 10% of the remaining membership agreement is required and will be charged to the card on file. All requests for membership cancellation must be submitted to Rejuvalase management at least ten days in advance. Once we have received and completed your request and cancellation fee, the next scheduled monthly charge and all those to follow will be canceled. Membership will be valid until, but not on, the date of the first canceled payment; after that date, all member benefits, including unused accrued services, will also be canceled. Cancellation requests are not retroactive. There are no refunds, and membership fees cannot be prorated if you cancel before your month is over. Once a monthly member cancels his/her Rejuvalase Membership, unused services will be forfeited, and the monthly Member may not rejoin for six consecutive months from the cancel date.

Waiver of Liability:  Members utilizing Rejuvalase services do so at his/her own risk. Rejuvalase and its management shall not be liable for any damages arising from personal injuries or damages sustained by Member in, on, or about the spa premises. Members assume full responsibility for injuries or damages and do hereby and forever release and discharge Rejuvalase and its owners, employees, affiliates, and agents from any claims, demands, damages, rights, or causes of action, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of the Member use, or intended use of the facilities and services performed thereof or this Agreement.
